Torbay and South Devon NHS Foundation Trust is seeking a Band 4 Nursing Associate/Assistant Practitioner to join the Alcohol Care Team at Torbay Hospital in Torquay. The role focuses on embedding our Alcohol Harm Reduction Programme and supporting admission avoidance and timely discharge.

You will work with clinical teams to optimize alcohol screening, deliver harm reduction training, and provide psychosocial support, with proximal supervision from a registered nurse/practitioner.

Qualifications

• Foundation Degree in healthcare is required.
• Care Certificate or equivalent.
• Knowledge or experience in Alcohol Services.
• Good written and verbal communication skills.
• Able to work to deadlines, prioritise and multi-task.

Responsabilités

• Embed robust alcohol screening and harm reduction in clinical settings.
• Screen inpatients to identify early alcohol-related harm.
• Use motivational interviewing to support recovery goals.
• Educate patients and families on alcohol use disorders and liver disease.
• Support admission avoidance and post-discharge follow-up.
• Coordinate Alcohol Care Team operations and triage referrals.
