Job summary

Berkshire Healthcare NHS Foundation Trust is seeking a Nursing Associate to support individuals with learning disabilities within Community Health services in Berkshire. You will work with a multidisciplinary team to deliver person-centred care, aiming to enable independence and reduce admissions.

The role requires NHS Foundation Trust values, a valid NMC registration, and commitment to ongoing professional development; travel across multiple sites is expected.

Qualifications

• Registered Nursing Associate on the NMC register.
• Evidence of Nursing Associate qualification.
• Evidence of Education to a minimum level 5 foundation degree specific to nursing associate practice.
• GCSE level 4-9 (Grade A-C) in Maths and English or equivalent.

Responsibilities

• Work as part of a designated clinical and care team delivering care that focuses on the direct needs of the individual.
• Carry out specific clinical and care tasks and responsibilities to a high standard under the direction of a registered LD Nurse.
• A full UK driving license and access to a car are essential to this post.

The role willpredominately bealigned to the East of Berkshire but cross cover as pressure and need arises. You will support individuals with Learning Disabilities, Autism and Complex needs to achieve improved health and wellbeing outcomes and greater independence within the Community. You will be an enthusiastic,suitablyskilledand experienced health care professional to undertake this role. You will support the ongoing development of our community-based specialist health services, for people with learning disabilities across Berkshire aimed at supporting people to remain in their communities andminimisingadmissions. • Work as part of a designated clinical and care team delivering care that focuses on the direct needs of the individual. Carry out specific clinical and care tasks and responsibilities to a high standard and competency, under the direction of a registered LD Nurse.
• Have the ability towork without direct supervision, at times delivering care independently in line with the individual's defined plan of care, within the parameters of practice of the Nursing Associate role, accessing clinical and care advice when needed. The must haves for this role 1. You should be self-motivated with excellentorganisationalskills and the ability to prioritize your workload.
• A commitment to supporting effective teamwork, with an excellent valuebasewill be crucial to your success in this role. 1. A key strength will be your ability to communicate effectively both verbally and in written form with the people we support and their families/carers, our staffteamsandthe multi-disciplinary team. 1. A full UK driving license and access to a car are essential to this post.
