A regional healthcare provider in Rotherham is seeking an enthusiastic Team Manager to lead their Community Ready Unit. This role involves providing operational leadership, enhancing patient flow, and collaborating with multidisciplinary teams to ensure effective patient care. The ideal candidate should have experience in acute nursing and discharge planning, along with registration with the Nursing & Midwifery Council.
We are looking for anenthusiastic, forward-thinking Team Manager to lead our Community Ready Unit(CRU) a key area supporting effective patient flow across the hospital. TheCRU provides short-term care for patients who are community ready and no longermeet the criteria to reside, ensuring they continue to receive safe,compassionate, and well-coordinated care while awaiting discharge or communitysupport.As Team Manager, you will provide strong, visibleleadership to the multidisciplinary team, fostering a positive culture thatprioritises patient experience, dignity, and timely discharge. You will beresponsible for the day-to-day management of the unit, ensuring safe staffing,effective communication, and efficient patient turnaround in line with Trustpriorities.Collaboration is central to this role. You willwork in close partnership with the Transfer of Care Hub (ToCH) Pharmacy, SiteMatrons, Therapy Teams, and external partners such as Age UK, and widerCommunity Services to deliver seamless care transitions and promoteindependence. Provide day-to-day operationalleadership for the Community Ready Unit, ensuring safe, effective, andcompassionate care for patients who no longer meet the criteria to reside.Support and drive patient flow by coordinatingtimely admissions and discharges, working closely with Site Matrons, thePatient Flow Team, and community partners.Maintain oversight of staffing levels, skill mix,and allocation of workload, ensuring a supportive and efficient workingenvironment for all team members.Lead effective multidisciplinary collaboration,developing strong working relationships with Pharmacy, Therapy Teams, CommunityServices, Social Care, and external organisations such as Age UK.Take a proactive role in Clinical Governance,promoting a culture of continuous learning, safety, and quality improvement.Participate in audits, incident reviews, and lessons learned processes toenhance standards of care.Monitor and improve performance through the use ofdata, audits, and quality improvement initiatives contributing to the Trustsmodern, forward-thinking approach to care delivery.Support professional development and wellbeing ofstaff, providing regular supervision, appraisals, and opportunities forlearning.Represent the Community Ready Unit at relevantTrust and system-wide meetings, promoting collaborative, patient-centredworking across services.
