A healthcare commissioning organization in Northampton is seeking a CHC Nurse Specialist to manage a clinical team and ensure high-quality patient assessments. The ideal candidate will demonstrate strong communication skills and the ability to lead in a multi-disciplinary environment. This role also involves supporting the Clinical Lead and adhering to NHS standards. The position requires a dedicated professional committed to providing exceptional patient care.
The post holder will manage the core clinical team on the delivery of assessments, ensuring they meet the key requirements of all KPIs, assessments are of the quality expected and support staff as required. All HR processes will be managed where appropriate for staff, clinical supervision and training needs.
The CHC Nurse Specialist will lead a team to deliver a professional and efficient service in line with the NHS Continuing Healthcare national framework and standards.
The post holder will have a flexible approach to care and will work across inter‑agency boundaries, maintaining good communication and liaison skills with all stakeholders. They will support the Clinical Lead to ensure that the work stream of programmes is planned and managed effectively and take the lead in assisting in their successful delivery.

The post holder will line‑manage a team of Continuing Healthcare Nurse Practitioners responsible for facilitating multi‑disciplinary assessments and patient reviews in line with standard operating policy and procedures. They will ensure that they and their team adhere at all times to the relevant National Framework and guidance, the NMC Code of Professional Conduct, and work within established policies, procedures, protocols and systems ensuring quality standards and performance as directed by the Specialist Nurse/Clinical Manager.
They will support the Clinical Manager to ensure that their workstream is planned and managed effectively and take the lead in assisting in its successful delivery.
