Nurse Practitioner

This is a unique opportunity to join a team of nurses and nurse practitioners to support a defined caseload of patients residing in care homes throughout Newcastle.

No two days are the same, you need to be adaptable and flexible to provide safe, evidence based holistic care.

We provide planned face to face clinical care and planned assessments responding to the changing needs of our patients.

Our nurses are involved in advanced health care planning with patients and their families with the aim of avoiding unnecessary hospital admissions. We also support end of life palliative care patients and patients with complex, long term conditions.

We work alongside members of the Multi-Disciplinary Team, supporting the linked GP's on weekly ward rounds, and the wider community teams. The team facilitates a weekly virtual Geriatrician M.D .T meeting with patients referred by health care professionals throughout the community.

We support a team of community nurses offering support and guidance.

The team also provides specialist advice and support to a range of qualified and non-qualified staff in the care home settings.

We are a dynamic team, working autonomously to assess patients who present with an acute clinical need and ensure patients with long term conditions are receiving appropriate care.

Applicants must have a full UK Driving License and access to a vehicle.

Main duties of the job

  • Essential nursing skills ( inc catheter care, wound care, end of life care, bladder scanning, phlebotomy).
  • Create and implement Emergency Health Care Plans with patients and their families.
  • Assist the GP with the weekly ward round in the care home.
  • New assessments, reassessments and reviews following hospital discharge.

Job responsibilities
  • The Newcastle upon Tyne Hospitals NHS Foundation Trust define a nurse practitioner as a qualified nurse where the primary purpose of role is to perform a specific technological procedure/s e.g. endoscopy, ultrasound in a defined patient group and/or advanced patient assessment and clinical decision making in a broad patient group.
  • To work towards the role of an independent nurse practitioner, contributing towards the delivery of a quality service matching clinical need to available resources within the focus of specialist nursing practice:
    • To ensure that the patient is the central focus of a streamlined patient journey ensuring optimal quality of care.
    • Will provide a prompt response to calls from colleagues, prioritising importance and clinical need.
    • Supports the clinical team to manage a defined caseload, activity to include assessment, investigation, diagnosis and treatment within defined protocols and training.
    • Provide immediate care management of patient including communication of plan to patients and significant others.
    • Review and refer to appropriate person, e.g. AHP, resident medical staff, specialist registrar or critical care outreach team.
    • Liaises with Patient Services Coordinator.
    • Communicate within multidisciplinary team, ward/departmental staff and/or out of hours team.
    • Contributes to specialist education and training to ward departmental staff, nursing and medical students.
Qualifications & Education
  • Registered Nurse (appropriate to branch)
  • Current NMC registration
  • Diploma plus specialist qualifications / experience within specialist area; prepared to take degree level study
  • Working towards / prepared to work towards:
    • Nurse Practitioner Qualification or equivalent
    • Clinical Skills Course
    • Non-Medical prescribing if relevant to role
    • Meets Nursing and Midwifery Council standards for student supervision and assessment (formally Mentorship) if relevant to role
    • Degree in health related subject
    • Has undertaken and completed Non-medical independent nurse prescribing course
Knowledge & Experience
  • Extended clinical practice underpinned by training and assessment e.g. venepuncture, cannulation skills and intravenous drug administration
  • Audit / research experience
  • Evidence of on‑going continuous professional development
  • Knowledge and understanding of relevant NHS policy or project specific policy context
  • Has understanding / experience of adult / child safeguarding
  • Research or audit experience
  • Evidence of contribution to education programmes and / or training
Skills & Abilities
  • Able to work independently and across disciplines as a member of a multi‑disciplinary team
  • Effective organisation and time management skills
  • Current driving licence if relevant to role
  • Change management skills and involvement in project work
