Job summary

Nurse Associate role based at Chippenham Community Hospital within HCRG Care Group's Bath Neighbourhood Service. This full-time post provides high-standard, person-centred nursing care to the district nursing caseload in the community.

You will join a multidisciplinary team, adhere to policies, pursue ongoing professional development, and safeguard privacy, dignity and confidentiality while supporting patients with a range of conditions.

Job Introduction

We are inviting applications for a motivated and skilled Nurse Associate to join our dedicated community services team based at ChippenhamCommunity Hospital. This is a full-time position offering a fantastic opportunity to provide high-standard, holistic, person-centred nursing care within the local community.

About the Role:

As a key member of our multidisciplinary team, you will contribute significantly to the assessment, planning, delivery, and evaluation of nursing care for patients on the district nursing caseload. You will operate with a breadth of knowledge across the lifespan, embodying the core values of nursing care: compassion, competence , communication, courage , care, and commitment .

  • Responsible for delivering a delegated workload against the needs of patients and service requirement

  • Organise, prioritise and adjust own workload in relation to patient care.
  • To work in care teams with other professionals, providing practical help to registered practitioners by delivering delegated aspects of nursing care to vulnerable and often highly dependent patients with a range of illnesses.

  • Registered as a Nurse Associate with the Nursing and Midwifery Council (NMC).
