A leading healthcare organization in Leigh on Sea is seeking an experienced Nurse or Allied Health Professional to join their Frailty Team. This role requires advanced clinical skills and a commitment to high-quality care for frail patients. The successful applicant will join the existing Frailty Team to provide expert advice, assessment and plans of care for frail patients within various departments and ward areas.

We are looking for an experienced Nurse/Allied Health Professional who is highly motivated and who has a commitment to the delivery of high quality and safe care to our frail patients. You will be organised and able to prioritise your own workload. You will be able to communicate effectively with staff at all levels of the organisation and have excellent leadership skills.
Mid and South Essex NHS Foundation Trust is the only acute hospital provider in the Mid and South Essex Integrated Care System (ICS), which is a system of health and care partners working together for our local population. Within Mid and South Essex there are 149 GP practices, operating from over 200 sites, forming 27 Primary Care Networks and one Ambulance Trust.
