A national health service organization in Moray is seeking a Band 5 Physiotherapist to join their Discharge to Assess team. This role offers a supportive environment for newly qualified physiotherapists, with a commitment to professional development and a focus on patient care. Band 5 Physiotherapist – Discharge to Assess (D2A)
Location: Moray Health & Social Care Partnership
Hours: 37 per week | Permanent

What is D2A?
The D2A team is therapy-led, operational 7 days a week since 2021, and works alongside social care colleagues in START (Short Term Assessment & Reablement Team) and a range of other services. Together, we support early discharge from hospital, enabling patient assessment in their own homes with a rehabilitation focus. The goal is to maximise safety and independence in daily living activities through focused, goal-oriented assessments that promote self-management and overall wellbeing.
This role is part of a wider expansion supported by Scottish Government funding to enhance Moray’s delivery of the NHS Grian Home First and Unscheduled Care Improvement programmes.
