Discharge to Assess (D2A) – Band 7 Occupational Therapist

0.5 WTE | 18.5 hours per week | Permanent

Are you an experienced Occupational Therapist ready to take the next step in your career?

We are seeking a dynamic and skilled Band 7 Occupational Therapist to join our evolving Discharge to Assess (D2A) service in Moray. This is a unique opportunity to play a pivotal role in shaping integrated care pathways as we blend operations with our START (Short Term Assessment & Reablement Team) colleagues—supporting earlier discharge and home‑based assessment. This post is one of several new opportunities that have arisen from additional funding from ScotGov for Moray Health & Social Care partnership to enhance its delivery of the NHS Grampian Home First and Unscheduled Care Improvement programmes.

  • Leadership & Influence: As a senior clinician, you’ll provide supervision to team members, contribute to service development, and influence strategic direction under the guidance of the Lead Occupational Therapist.

  • Collaborative Innovation: Work within a therapy‑led, multidisciplinary team including OT, PT, Nurse Practitioners, Therapy Support Workers, and Social Care Assistants—delivering goal‑oriented, person‑centred rehabilitation in patients’ homes.
  • Quality & Improvement: Be part of a service that values evidence‑based practice, quality improvement methodology, and continuous evaluation to enhance patient outcomes.
  • Supportive Environment: Join a well‑established OT team with robust supervision, mentoring, and audit frameworks, and strong links across NHS Grampian and national networks.
  • Wider Occupational Therapy network: community, acute, paediatric and primary care services—offering opportunities for peer support, supervision, and professional growth.
