Job summary

Qualifications

Your role involves participating in the day‑to‑day delivery of services, assisting with planned activities, and supporting individuals to promote greater choice, independence, and inclusion. You will contribute to individual assessment, planning, and review processes, ensure the safe management of risk, administer medication as required, and maintain effective communication with all parties involved. You will be working within various community‑based settings, including day services, supported accommodation, and short breaks, adhering to company policies and procedures. You are expected to work flexibly and may be required to work evenings, weekends, and holidays.

You are someone passionate about supporting individuals with learning disabilities, physical disabilities, autism, or associated mental health conditions. You operate with a big heart, empathy, and kindness, and you value trust, transparency, and teamwork. You hold a Diploma in Health & Social Care Level 2 or equivalent, and have experience working with vulnerable people in community or residential settings. You are able to demonstrate behaviors aligned with our core values and are committed to personal and professional development.
