An exciting opportunity has arisen for a Band 6 Neuro-Developmental Nurse / Senior Learning Disabilities Nurse. You will have a passion for working with children and young people and will work withina team that provides support and management of children with severe or complexlearning disabilities or neuro-developmental disabilities including but notlimited to ASD, ADHD, and global-developmental delay.
The team deliver services tochildren and young people across the borough of Halton (Widnes and Runcorn). The vacancy is a full-timerole 37.5hours however, the team would welcome applications from nurses who maywish to work on a part time or job share basis. The Child Development Centre offers free parking. Mileage and toll bridge expenses can be claimed for duties requiring a crossing of the bridges.

Job Purpose:
To work as part of a Neurodevelopmental Nursing Team (NDNT) that provides support and management of children with severe or complex learning disabilities or neurodevelopmental disabilities including but not limited to Autism, ADHD, and global-developmental delay.
To work closely with community paediatricians, children and young people, parents/carers, general practitioners, schools and other partners to develop a greater understanding of the conditions and facilitate appropriate support for children, young people and their families.
The post holder will work autonomously whilst being part of a nursing team that provides a community-based service undertaken in the Halton Child Development Centre, GP surgeries and other clinical centres, Family Hubs and at home.
The post holder will be an experienced nurse with strong clinical leadership and demonstrate an ability to further develop service provision in this area of clinical practice.

Duties and Responsibilities:
Clinical
To assess, plan and implement packages of care for children & young people, up to 19 years of age, with a diagnosed severe or complex learning disability or neurodevelopmental disability (including attention deficit hyperactivity disorder, autism).
To assess the needs, including challenging or complex behaviour and sleep difficulties, of a child/young person, and plan, implement and evaluate individualised care in collaboration with the child/young person, family, and other involved agencies/professionals.
To provide advice, support and information to the child/young person and their family at the early stages of diagnosis, when needs change or during transition periods; also signposting and/or referring to other sources of support including voluntary and community assets.
To provide skills and strategies for young people/parents or carers to use effectively to help manage their own/their child's long-term condition.
To promote the health and well‑being of children and young people at every contact.
To have an understanding and knowledge of medications prescribed for ADHD and/or sleep difficulties; supporting medication concordance to improve outcomes for children and young people.
To develop care pathways in line with NICE guidance as appropriate to the development of the service and in consultation with all key stakeholders.
To run nurse-led clinics; demonstrating the ability to work as an autonomous practitioner including the recording of clinical observations, the ongoing assessment of treatment effectiveness, monitoring compliance with clinical practice and NICE guidance.
To offer joint working and specialist advice to other members of the multidisciplinary team and external agencies to implement care plans, packages of care and to evaluate outcomes.
To liaise and share information with other agencies, with appropriate consent and in accordance with data protection guidelines, in order to support the needs of the child or young person safely and effectively.
To develop, run and evaluate group support for children, young people with severe or complex learning disability or neurodevelopmental disability and their families/carer.
To ensure that clients are actively involved in all care planning and decision making as per Fraser Guidelines and Mental Capacity Act, 2005.
To contribute to statutory processes such as Education Health & Care Plans as required.
To use high level communication skills throughout all duties within the clinical role including verbal, non‑verbal and written formats.
To make appropriate clinical decisions following assessment; seeking support and advice from a peer, manager or named safeguarding nurse as appropriate.
Where appropriate, to support families through transition processes from pre‑school to education, primary to secondary school and secondary to adult services.
To discharge from the service appropriately, agreeing a point of closure with the young person, parent or carers and informing relevant professionals. Alternatively following Trust policy for children who are not brought for health appointments.
Managerial
To show a strong commitment to team working, demonstrating respect and consideration to other team members and colleagues in partner agencies.
To attend and provide leadership in team meetings and confidently represent the service at relevant Trust and external partner meetings.
As a senior nurse act as deputy to the Team Leader when appropriate and offer direction, support and supervision to the skill mix team.
To effectively manage a caseload, prioritise clinical demands, delegate appropriately to other members of the team and act as a care coordinator for the service.
To be able to identify, manage and review risks undertaking comprehensive risk assessments when necessary.
To deal with concerns and complaints sensitively, avoiding escalation where possible, inline with Bridgewater Trust policy.>
To learn from incidents, complaints and user feedback to improve the quality of the service delivery.
To understand service/team key performance indicators and contribute to the accurate and timely collection of statistical data.
To participate in departmental clinical audit and research as part of a process of continuous quality improvement.
Leadership
To lead and manage quality improvements, encouraging others to recognise the need for change, developing a shared vision, inspiring and motivating other team members.
To provide expert clinical leadership by engaging and contributing to the development of the team and the service.
Contribute to the development and implementation of policies, procedures and guidelines relevant to own area of work.
To adhere to the Codes professional standards and behaviour for nurses as required by the Nursing & Midwifery Council (NMC).
To develop a working knowledge of relevant procedures including: safeguarding children, SEN procedures, working with vulnerable adults and other legal frameworks.
Education & Teaching
To develop, deliver and evaluate tailored awareness training for ADHD, Autism and other related conditions to members of the multidisciplinary team and partner agencies.
To develop, deliver and evaluate group training to young people, parents/carers for ADHD, Autism and other related conditions.
Act as mentor/preceptor to new staff and participate, where appropriate, in developing the skills and potential of colleagues.
To contribute to the development of parent/client information ensuring this is relevant, accessible and up to date in a written and digital format.
Continuing Professional Development
To participate in individual performance review, ensuring that the objectives set reflect the service and organisational plans.
To identify personal/professional development within a performance review; to attend relevant training, undertake development activities in order to maintain and enhance skills and knowledge required to maintain up to date registration.
To demonstrate the ability to reflect on practice with peers, clinical supervisor, manager to identify own strengths and development needs.
To attend and engage in clinical and safeguarding supervision.
To use evidence-based practice, maintain skills and knowledge in accordance with current evidence based research, best practice and Trust policies/guidance.
Record Keeping/Records Management
To maintain up to date written records in line with professional and organisational requirements.
To maintain strict confidentiality for all clients according to Trust policy.
To maintain record keeping skills and knowledge by undertaking regular training and participating in the Trust wide record keeping audit.
To adhere to Bridgewater and local service guidance in relation to the management of Records, including storage and transfer.
Physical
To demonstrate knowledge in moving/handling clients with disabilities.
To use a range of techniques appropriate for those with severe and profound disabilities including symbols systems, alternative communication equipment and other resources in the provision of support to families.
