Qualifications

• Registered Nurses from Adult, Mental Health or Learning Disability backgrounds required.
• Confidence in representing the organization at meetings.
• Ability to support clinical pre-admission assessments.

Responsibilities

• Support care homes with clinical assessments.
• Produce individual cost per care assessments.
• Coach Clinical Nurse Managers in the assessment process.
• Support with referrals and build relationships with referrers.
