Job summary

Sussex Partnership NHS Foundation Trust invites applications for a Mental Health and Wellbeing Practitioner in Eastbourne. The post sits within the Eastbourne NMHT and works closely with GP practices, NHS Talking Therapies and VCSE partners to deliver triage, assessments and early interventions.

The role involves coordinating triage, conducting brief interventions and signposting to appropriate services, while building strong links with stakeholders and contributing to service development across

Qualifications

• Must be a registered Mental Health Professional (MHN, social worker or occupational therapist).
• Excellent skills in assessment and risk assessment; strong communication abilities.
• Experience working with GP practices and NHS teams is advantageous.

Responsibilities

• Triaging referrals into the NMHT and NHS Talking Therapies (NHS TT).
• Conducting timely assessments and brief interventions.
• Making onward referrals to Secondary or Urgent Care as needed and signposting.
• Liaising with GP practices, VCSE partners and other stakeholders.
• Contributing to development of triage hubs and the Neighbourhood model.
