Overview

Pennine Care NHS Foundation Trust is advertising a permanent, full-time opportunity for a Mental Health and Wellbeing Practitioner to join the Neighbourhood Mental Health team working across Stockport Primary Care Network/ Neighbourhood Mental Health Teams.

This role supports the transformation of community mental health services for adults and older adults, bridging primary care, secondary mental health services, and physical healthcare.

You will be employed by Pennine Care NHS Foundation Trust but will work within the Stockport Primary Care Network, based in local GP practices and Living Well Hub. You will be a visible and accessible point of contact for GPs and service users, providing timely support and intervention for individuals with mental health needs.

The focus is on early assessment, brief psychological interventions, and helping individuals access appropriate community and mental health services. You will help people identify achievable goals, promote self-management, and connect them with local resources.

Qualifications and experience
  • Education and Qualifications — Essential: Successful completion of the national curriculum for Mental Health and Wellbeing Practitioners (Adult Specialist Mental Health) (equivalent to degree level); academic/accredited qualifications in mental health; postgraduate certificate (RQF Level 7); graduate certificate (RQF Level 6).
  • Knowledge and Experience — Essential: Experience working with people with mental health needs gained through graduate or postgraduate training with supervised practice; experience running groups/activities; experience working as part of a team; ability to analyse and communicate complex information verbally and in writing.
  • Skills and Competences — Essential: Ability to establish empathic, supportive relationships with people in distress or cognitive impairment, their families and carers; ability to handle confidential information; ability to communicate sensitively and with empathy; ability to utilise clinical supervision; ability to liaise with other teams and external agencies as required for wellbeing of service users.
  • Work related circumstances — Essential: Use of a car or access to mobility to travel across the Trust footprint; willingness to perform duties in line with Equal Opportunities and Diversity policies; enhanced DBS disclosure required for regulated activities.
