Are you a dedicated Mental Health Practitioner looking for a rewarding role where your expertise truly matters? Our West Lancashire Community Mental Health Team (CMHT) is seeking a Band 6 professional to join us on a full‑time basis.

Overview

This is your opportunity to manage a diverse caseload, providing essential assessment, planning, and review for service users with complex mental health needs. As a vital member of our multidisciplinary team, you will champion person‑centred care pathways and make a tangible impact on individual recovery and well‑being within the community.

We are looking for an experienced professional with strong skills in risk assessment and management, and a passion for collaborative working. A full driving licence is essential.

Main Duties
  • Hold responsibility for a defined caseload, providing expert assessment and crafting personalised care plans for adults with complex mental health conditions.
  • Deliver evidence‑based interventions, regularly review progress, and coordinate effectively with other health professionals to ensure seamless support.
  • Conduct thorough risk assessments and develop robust management plans to safeguard service users.
  • Provide community‑based care, travelling to meet service users in their homes and local settings.
Qualifications & Experience
  • Relevant professional qualification (e.g. RMN, Social Worker, OT) with current UK registration (NMC, SWE, HCPC).
  • Significant post‑qualification clinical experience working with service users experiencing a range of mental health problems.
  • Experience assessing, planning, implementing, and reviewing care in a community setting.
  • Demonstrable knowledge and experience in clinical risk assessment and management.
  • Ability to manage a complex caseload autonomously with effective prioritisation.
  • Excellent verbal and written communication skills, with the ability to build therapeutic relationships.
  • Full valid UK driving licence and access to a vehicle for community visits.
  • Experience working within a Community Mental Health Team (CMHT).
  • Additional training in a therapeutic modality (CBT, DBT, Family Intervention) is desirable.
