We are looking for an experienced professional with strong skills in risk assessment and management, and a passion for collaborative working. A full driving licence is essential.
Main Duties
- Hold responsibility for a defined caseload, providing expert assessment and crafting personalised care plans for adults with complex mental health conditions.
- Deliver evidence‑based interventions, regularly review progress, and coordinate effectively with other health professionals to ensure seamless support.
- Conduct thorough risk assessments and develop robust management plans to safeguard service users.
- Provide community‑based care, travelling to meet service users in their homes and local settings.
Qualifications & Experience
- Relevant professional qualification (e.g. RMN, Social Worker, OT) with current UK registration (NMC, SWE, HCPC).
- Significant post‑qualification clinical experience working with service users experiencing a range of mental health problems.
- Experience assessing, planning, implementing, and reviewing care in a community setting.
- Demonstrable knowledge and experience in clinical risk assessment and management.
- Ability to manage a complex caseload autonomously with effective prioritisation.
- Excellent verbal and written communication skills, with the ability to build therapeutic relationships.
- Full valid UK driving licence and access to a vehicle for community visits.
- Experience working within a Community Mental Health Team (CMHT).
- Additional training in a therapeutic modality (CBT, DBT, Family Intervention) is desirable.
