A leading health organization in Trafford seeks a Mental Health Practitioner to join their multidisciplinary team at the Old Trafford Wellbeing Centre. This role involves assessing and formulating mental health needs, triaging referrals, and working with various agencies. Competitive salary ranges from £38,682 to £46,580 annually. This post is for a Mental Health Practitioner for the Trafford Mental Health Neighbourhood team (Living well).
This post will be based at the Wellbeing Centre at Old Trafford and will cover the North of Trafford, but the expectation is that the practitioner will move to other hubs when needed for the service needs, the other hubs are in Central, West and South Trafford.
The postholder will undertake a key worker role, alongside an assessment and formulation role within the team. The Living Well Service enables timely access to mental health services when people need it most and provides support for people who might not traditionally meet the threshold or criteria of existing mental health services. The team is multidisciplinary and is operated in partnership with Bluesci, our third sector partner.
You will need to have experience of triaging and signposting referrals as well as being able to liaise with multitude of agencies and professionals. Experience and knowledge of community services and third sector organisations in Trafford will be beneficial for this role.
Trafford Neighbourhood team (Previously Living well Service) is compassionately-led, trauma-informed, and recovery-focused. CPD opportunities are available and personal interests/skills are supported to develop and grow.
