Job summary

Lancashire and South Cumbria NHS Foundation Trust (LSCFT) is seeking a Senior Mental Health Practitioner to join the Fylde Coast Home Treatment Team. The role focuses on rapid assessment, risk management and delivery of intensive community-based care for individuals in mental health crisis.

Qualifications

• Recognised Core Mental Health Professional Qualification (e.g. RMN/Dip SW/OT).
• Experience of post qualification clinical work with service users experiencing a range of mental health problems.
• Experience of delivering evidence based interventions in a time-limited framework.
• In-depth knowledge of mental health problems and related risk and protective factors.

Responsibilities

• Provide a fast, responsive service to assess individuals’ needs for access into mental health services.
• Undertake Mental Health Assessments including Risk Assessments and provide rapid holistic assessment and sign posting.
• Support with the role of reviews within crisis house and crisis lounge on a rostered basis.
• Refer to other clinicians or services/agencies where necessary.
• Provide managerial support to the team in the absence of the Team Manager.
• Organise and deliver rapid intensive intervention to meet health and social needs during a serious mental health crisis.

• Specialist mental health inpatient care for individuals with learning disabilities, alongside specialist community support for children and adults with learning disabilities and autism, including intensive support.
• Extensive community physical health and well-being services for children and adults, covering prevention, treatment, rehabilitation, and long-term condition management.

The Trust was first established in 2002 and employs approximately 7,500 staff who provide care from more than 400 sites. The organisation offers opportunities for medics, mental health, general nurses, childrens' nurses, allied health professionals, psychology, administration and clerical staff, apprentices and volunteers, as well as those specialising in learning disabilities and community physical health services.

This is an exciting opportunity for a senior mental health practitioner to join the Fylde Coast Home Treatment Team.
The Fylde Coast Home Treatment Team provides 7day support to service
users who are suffering from mental health concerns that would otherwise require admission to an acute in patient environment. There is a strong focus on a recovery model and effective risk management to provide high quality care in a community setting.

The Fylde Coast Home Treatment Team are always striving towards

achieved. Currently the team is undergoing an exciting period of change that will see the working together to develop a model of care that ensures effective service delivery in partnership with local services and across the Urgent Care Pathway.

The successful applicant will be dynamic and forward thinking they will be passionate about working with service users with acute mental health
concerns and providing a recovery focused approach. The successful
applicant will have the ability to work as part of a Team and in partnership with local services.

development needs

Main duties of the job

• To provide a fast, responsive service to assess individuals needs for access into mental health services
• To undertake Mental Health Assessments including Risk Assessments, providing rapid and holistic assessment, care and sign posting of individuals presenting with other urgent risk issues.
• To support with the role of reviews within out crisis house and crisis lounge on a rostered basis
• To be proficient in referring to other clinicians within the service and/or referring to other services/agencies where necessary.
• To provide a "gatekeeping" function as detailed in the Operating Procedure.
• To provide managerial support to the team in the absence of the Team Manager
• To organise and deliver a rapid intensive intervention service in order to meet the health and social needs of people experiencing a serious mental health crisis
• To form close working relationships with and provide highly specialised advice and information on mental health issues to a variety of other services/agencies/organisations and carers both within and outside mental health services.
• To participate in and contribute to local and National strategies to improve services.
• To carry out clinical supervision and appraise and produce development plans with lower grade staff members.
