Job summary

OneMedical Group in Aberdeen is seeking a compassionate Mental Health Practitioner to support primary care teams across two Aberdeen practices. The role involves mental health assessments, risk management and brief interventions within a multidisciplinary team, linked with GPs and local services.

The successful candidate will deliver person-centred care, maintain records and refer patients to relevant services, contributing to improved access to mental health support in the community.

Qualifications

• Assess mental health needs and risks for patients.
• Develop person-centred care and safety plans.
• Deliver brief interventions and provide ongoing support.
• Offer specialist guidance to GPs and the MDT.
• Refer to community and secondary care services as needed.

• Undertake comprehensive mental health and risk assessments.

We are looking for a compassionate and experienced Mental Health Practitioner to support our Primary Care teams across our two Aberdeen practices; Great Western Medical Practice and Whinhill Medical Practice.

This is an exciting opportunity to work as part of a multidisciplinary team, providing timely mental health assessment, risk management and brief interventions for patients presenting with a wide range of mental health needs. You'll work closely with GPs and other healthcare professionals to improve access to mental health support, promote early intervention and ensure patients receive the right care, at the right time.
