Are you passionate about supporting people with their mental health and helping them access the right support at the right time? We are looking for a compassionate and motivated Mental Health Pathway Facilitator to join Portsmouth Mental Health Hub.
In this rewarding role, you will be the first point of contact for individuals seeking mental health support. Working autonomously, you will undertake person-centred telephone assessments, identify risks, develop collaborative care plans, and connect people with appropriate mental health, housing, financial, community and wellbeing services. You will work closely with colleagues across primary care, mental health services and voluntary sector organisations to support recovery and prevent mental health crises.
We are seeking someone with excellent communication and listening skills, experience working with service users, carers or patients, and a genuine commitment to person-centred care. Knowledge of mental health and risk management is essential, as is the ability to work flexibly in a fast-paced environment.
In return, you'll join a supportive team committed to Compassion, Accountability, Respect and Excellence, with opportunities for training, development and career progression.
Closing the gap between need and support, you'll help people find the right pathway to recovery.
