An NHS Trust in England is seeking a qualified Mental Health Nurse Practitioner for its Memory and Dementia Community Team. The successful candidate will provide holistic care to patients experiencing dementia, perform assessments, and work collaboratively with a multidisciplinary team. A non-medical prescribing qualification is essential, along with relevant experience in therapeutic interventions and community settings. This role offers training and development opportunities within a supportive environment.

An opportunity has arisen to recruit a band 7 mental health Nurse Practitioner within the Memory Assessment Pathway, based in Lichfield/Burntwood and Mercian PCN.
Applicants should possess the knowledge and skills to enable them to work in the memory and dementia speciality, working with service users affected by all types of dementia, including challenging and complex needs associated to a dementia diagnosis. Applicants must have non‑medical prescribing qualification.
Applicants will be required to work closely with the existing multi‑disciplinary team members, providing diagnostic assessment and follow ups to those referred. The role will include close working with physical and social care colleagues along with our GP and primary care colleagues.
