Qualifications

• Will be required to complete breakaway/de-escalation training.
• Able to meet the travel requirements of the post.

Responsibilities

• Completing assessments on patients referred within the Redditch locality.
• Holding a caseload of patients with various mental health illnesses.
• Developing care plans with patients and the MDT.
• Risk assessing and managing patients based on their needs.
• Participating in the duty system to assist patients.
• Supervising and supporting junior team members.

Skills

Experience of working in a Mental Health setting

Assessment of people presenting with mental health problems

Assessment and management of clinical risk

Working within a multi-disciplinary team

Managing own case load

Working in a primary care setting

Experience of Care Coordination

Experience of delivering evidence-based psychosocial interventions

Education

Relevant professional qualification in Health or Social Care

RMN, BSc/DipSW, BSc OT

Teaching/training qualification

Psychological therapy qualification
