Mental Health Blended Case Manager (Awaken Mental Health Case Management)

Awaken Mental Health Case Management is seeking case managers to join our growing team. We offer a competitive salary and an excellent benefit package, including mileage reimbursement. As a community-based mental health blended case manager, you will be part of a compassionate team dedicated to providing top-notch care to those in need.

Your role will involve working closely with adults with mental health disorders to assess their mental health needs and develop personalized goals and service plans to help them live independently. Responsibilities
  • Conduct mental health assessments for clients in various locations, including homes and community centers.
  • Develop personalized service plans in collaboration with clients to address their mental health needs and goals.
  • Coordinate with healthcare providers, social services, and community resources to ensure comprehensive care for clients.
  • Maintain accurate and up-to-date documentation of client progress and treatment plans.
  • Maintain a productivity rate of at least 58%.
Qualifications
  • 1-2 years' experience in mental health case management.
  • Bachelor's degree in psychology, social work, or related field.
  • Strong interpersonal communication skills.
  • Knowledge of mental health resources and services in the local area.
  • Valid driver's license and reliable personal vehicle.
  • Bi-lingual individuals preferred but not required.
Compensation

$22 - $24 hourly

The Awaken Mental Health Case Management program (*A Division of TASC) provides support to adults with severe mental illness throughout Berks County. The program supports individuals with mental illness to attain access to medical, social, educational, and other services to promote a healthier and more stable lifestyle-encouraging a more independent living lifestyle. Awaken provides mental health case management as approved and overseen by OMHSAS/Community Care Behavioral Health (CCBH) standards. Medicaid/Community Care Behavioral Health Approved.

Through the Awaken Mental Health Case Management program, TASC as an intervention agency, continues to grow and reach an increasing number of individuals in need. Together, through its many drug and alcohol service units, the 60+ employees working at TASC help thousands of individuals overcome the devastating effects of mental illness and substance abuse each year.
