Pennine Care NHS Foundation Trust is pleased to advertise a permanent, full‑time opportunity for a Mental Health and Wellbeing Practitioner to join the team working across the Heywood, Middleton and Rochdale (HMR) Primary Care Network/Neighbourhood Mental Health Teams. The role is a key part of the ongoing transformation of community mental health services for adults and older adults, helping to bridge the gap between primary care, secondary mental health services and physical healthcare.
