Job Summary

Kirklees Memory Assessment Service is recruiting a Band 5 nurse to work 37.5 hours per week at Folly Hall in Huddersfield, covering North and South Kirklees. The full-time post supports a multidisciplinary team of Memory Nurses, Advanced Nurse Practitioners, Social Care Professionals and Occupational Therapists with access to Dietetics and Psychology.

The Service has achieved third MSNAP accreditation and a Sustainable Mental Health Service Commendation. All employees are strongly encouraged to be fully vaccinated against COVID-19.

Main Duties

As a Band 5 nurse the candidate will:

  • Undertake initial assessments of service users and formulate care plans.
  • Provide intervention work, risk management and clinical decision‑making in line with leadership.
  • Collaborate with the team to ensure seamless service delivery.
  • Participate in reflective practice, nursing research and clinical supervision.
  • Attend the team duty rota and manage enquiries and direct referrals in accordance with the Operational Policy.

Experience of working within community settings is advantageous. A full UK driving licence and use of own car are essential.

Person Specification

Essential

  • Registered Mental Health Nurse with current UK professional registration.
  • Assessment and care‑planning skills; risk assessment and risk management.
  • Competence in all aspects of care and risk management within the Care Programme Approach.
  • Good communication skills, ability to work independently and in a multidisciplinary team.
  • Current driving licence and access to a car during working hours.

Desirable

  • Post‑basic qualification and training relevant to memory assessment.
  • Experience of mentoring or supervising students/trainees.
  • Knowledge of Mental Health Act, Mental Capacity Act, Health & Safety, Care Act, Safeguarding, and clinical governance.
  • Evidence of using IT systems and audit processes.
  • Awareness of cultural diversity and ability to incorporate it into care planning.
