Job summary

Qualifications

• Qualified pharmacy technician with registration to GPhC.
• Experience supporting medicines optimisation in primary care is desirable.
• Ability to work as part of a multidisciplinary team in community and hospital settings.

Responsibilities

• Undertake patient-facing medicines optimisation tasks and SMRs.
• Support medicines reconciliation during transitions of care.
• Contribute to medication reviews and safety searches.
• Support PCN in reducing wastage and implementing eRD where applicable.
