About the Role

We are looking for three highly motivated Pharmacy Technicians to improve Medicines Optimisation across Greater Manchester:

  • 2 positions in Trafford – 1 with a GP Practice focus and 1 with a care homes focus
  • 1 position in Wigan – GP Practice and care homes focus
The post holder will promote and facilitate safe, effective, high‑quality and cost‑effective prescribing and medication management in the base locality and across Greater Manchester.
Key Responsibilities
  • Provide technical assistance in GP practices to implement and maintain high‑quality, cost‑effective changes in prescribing patterns and reduce medicine‑related risks.
  • Act as a medicines optimisation link person for specified GP practices and care homes, implementing initiatives in line with primary care and medicines optimisation priorities.
  • Support the delivery of the medicines optimisation work plan in GP practices.
  • Travel across the locality, review prescribing within GP practices and make evidence‑based recommendations to improve patient outcomes in clinical areas focusing on quality, safety and waste.
  • For care‑home‑focused roles, regularly travel to and work on‑site at care homes.
  • Liaise with secondary care providers, community pharmacies, care homes and other stakeholders as required to implement medicines optimisation.
  • Identify, implement and deliver on medicines optimisation, primary care quality and QIPP objectives.
  • Influence and change prescribing habits of prescribers in primary and secondary care.
  • Support locality and GM medicines optimisation teams to ensure effective implementation of work programmes, strategies and priorities.
  • Develop, implement and complete clinical and non‑clinical audits to support medicines optimisation objectives and improve prescribing quality.
  • Provide information, advice and query answering in relation to general medicines management issues in line with national and regional guidance.
  • Use prescribing data, knowledge and technology to contribute to the management of the prescribing budget and support a portfolio of medicines optimisation projects, services and initiatives.
  • Participate in the training of GP practice staff, community pharmacy staff and other multidisciplinary health and social care professionals, where appropriate.
  • Support local authorities to improve and/or assure medicines optimisation within care homes and for patients receiving care at home.
  • Provide technical and clinical advice to multidisciplinary teams, including as part of safeguarding investigations.
  • Carry out medication optimisation reviews in domiciliary, care home or clinic settings, meeting with vulnerable patients and carers referred by other healthcare or social‑care professionals or identified during audits.
  • Communicate effectively within multidisciplinary teams and support and reassure patients when necessary, including having difficult discussions concerning sensitive issues after assessing risk and potential harm associated with medication.
  • Work with wider teams and contribute on a multi‑disciplinary and multi‑agency basis to overall health‑care quality improvement and the health inequalities agenda, providing medicines‑related advice to support improved patient care pathways.
