Salary: £25,065 - £26,734 (Full-Time Equivalent)
Contract Type: Permanent (with opportunity for overtime via Bank Shifts on weekends)
Hours: Full Time | Monday to Friday (weekend shifts available)

About the Role

We are currently recruiting for a Care Navigator to support Clinical Services and Urgent Care teams in York. This is a non-patient-facing role, ideal for someone organised, compassionate, and experienced in healthcare administration. You'll play a key role in ensuring patients receive timely, high-quality care through effective coordination and administrative support.

  • Deliver high-quality administrative support including appointment booking and referral processing
  • Use SystmOne to manage patient data accurately and confidentially
  • Respond to patient and professional enquiries with professionalism and compassion
  • Support the coordination of care across services and follow up on patient needs
  • Work flexibly across teams to support service delivery
Requirements
  • Smart Card - Essential
  • Previous experience in one or more of the following roles: GP Receptionist, GP Administrator, Medical Receptionist, Healthcare Assistant, Patient Coordinator
