Job summary

Hamble Valley Health is seeking a GP to cover maternity leave for 12 months. The role involves 6 sessions per week across 3 days, with a base site at Blackthorn Health Centre and occasional work at Hedge End Medical Centre and Bursledon Surgery.

You will manage a patient list with responsibilities including prescribing, pathology results, and care planning for a list of around 37,000 patients, operating on a rolling rota as clinical lead for urgent care when needed.

Qualifications

• GMC Registration is essential.

• DBS check is mandatory.

Responsibilities

• Provide surgery consultations, telephone consultations, and home visits as required.
• Make autonomous decisions in line with practice protocols.
• Assess health care needs of patients with undiagnosed problems.
• Screen for disease risk factors and early signs of illness.
• Develop care plans and provide counselling and health education.
• Prescribe in accordance with the practice formulary and manage caseload.

We are looking for a maternity cover, this will be for 12 months, 6 sessions per week across 3 days, which would include a holding list.

The Practice iscommitted to delivering high-quality patient care which requires us to have asupportive and cohesive team.

The location will be acrossHedge End Medical Centre, Blackthorn Health Centre, and Bursledon Surgery.

We offer Termsand Conditions equivalent to the BMA model contract. We are happy to considernewly qualified GPs as well as experienced GPs.

Main duties of the job

This post has a base as their main site at Blackthorn Health Centre. On a rolling basis, you will also work in our Urgent Care Centre which is atBursledon Surgery. This is on a fair share rota. In the same way, you will beour "operational director" around once every 3-4 weeks. This meansyou are the clinical lead for the urgent care centre in the practice. Thisrequires a longer working day (8am to 6.30pm).

Routine care is generally 13 appointments in the morning and 12 in theafternoon. Mornings are generally 8am-1pm, and afternoons 2pm-6pm. We are asingle practice Primary Care Network. We have visiting paramedics, so apartfrom terminal care, visits are rare. You will be responsible for theprescriptions, pathology results and incoming correspondence for your listsize. On average, you will manage around 40 prescriptions a week, 20 results aweek, and 5 letters a week per session. We have robust processes in place tosupport prescribing and document workflow.

BlackthornHealth Centre and Hedge End Medical Centre have merged and are now known asHamble Valley Health. Our list size is over 37000, and our patients are mainlyconcentrated in Hamble, Bursledon, Hedge End and Botley. Our aim is to providehigh quality, accessible medical care to our patients, whilst maintaining asupportive and pleasant working environment for clinicians and staff.

We are part ofHampshire and Isle of Wight ICB and offer General Medical Services to ourpatients.

We are a training practice and take registrar doctors and also teach medical studentsfrom Southampton School of Medicine.

The practicehas seven Partners. The current Partnership team is Dr Harris, Dr Cropley, DrGraham, Dr Sonpal, Dr Shahsavanspour, Dr Yates and Dr McKenzie.

We employfourteen salaried GPs, five Advanced Nurse Practitioners, a Nursing team offifteen, and a reception/admin team of seventy.

The practiceuses Emis as a clinical system. We are paper light, so scan all incomingcorrespondence onto patients medical records.

Job responsibilities

• In accordance with the practice timetable, as agreed, the post-holder will make him/her-self available to undertake a variety of duties including surgery consultations, telephone consultations and queries, visiting patients at home, checking and signing repeat prescriptions and dealing with queries, paperwork and correspondence in a timely fashion
• Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation
• Assessing the health care needs of patients with undifferentiated and undiagnosed problems
• Screening patients for disease risk factors and early signs of illness
• In consultation with patients and in line with current practice disease management protocols, developing care plans for health
• Providing counselling and health education
• Admitting or discharging patients to and from the caseload and referring to other care providers as appropriate
• Recording clear and contemporaneous consultation notes to agreed standards including appropriate coding and use of electronic templates and other electronic systems as agreed.
• Compiling and issuing computer-generated acute and repeat prescriptions (avoiding hand-written prescriptions whenever possible)
• Prescribing in accordance with the practice prescribing formulary (or generically) whenever this is clinically appropriate
• In general the post-holder will be expected to undertake all the normal duties and responsibilities associated with a GP working within primary care.
