Overview

The post holder will be a SCPHN Qualified School Nurse, who will provide specialist advice and evidenced based practice to Clients and Schools in Enfield to ensure that Children and Young People with Long Term Health conditions are well supported and have a safe environment at school. Influencing the work undertaken by the wider school nursing workforce. The School Nurse for Long Term Conditions will be expected to contribute towards identifying gaps, good practice and service development opportunities for inclusion in the annual workplan for long term conditions. The School Nurse for Long Term Conditions will be the nurse leading on the implementation of the annual work plan for long term conditions, and will participate in partnership meetings to improve Enfield’s response to long term conditions and attend meetings and forums to promote initiatives in the annual plan. The initial focus will be on improving the management of childhood asthma by leading on the implementation of an Asthma Friendly Schools initiative in Enfield, to support children, young people, families and schools to improve the management of asthma and reduce the number of school days missed due to asthma.

Responsibilities

• Lead on the annual work plan for long term conditions and implementation thereof.
• Participate in partnership meetings to improve Enfield’s response to long term conditions and attend meetings and forums to promote initiatives in the annual plan.
• Focus on improving the management of childhood asthma by leading on the Asthma Friendly Schools initiative to support pupils, families and schools and reduce school days missed due to asthma.
• Develop Enfield School Nursing Service’s approach to other long term conditions (eg allergies, epilepsy, eczema, sickle cell and diabetes).
• Support the 0-19 Service Team to achieve Asthma Friendly Accreditation in Enfield schools.
• Work in partnership with the 0-19 School Nursing and Health Visiting Teams, service users and stakeholders to promote the health and well-being of Children and Young People aged 5-19 and their families in the school environment, through effective delegation, workload management and monitoring of care standards.
• Identify and monitor health needs of a given population using profiling tools and facilitate the implementation of evidence-based care.
• Link with schools and family hubs to promote, advise and guide current best practice and NICE guidelines (eg Asthma Friendly Schools; Sickle Cell guidance; Anaphylaxis best practice).
• Agree an annual work plan for long term conditions with the Service Lead for School Nursing to inform the workstreams for the year ahead.
• Promote evidence-based practice, NICE guidelines, Government Directives, NHS Plans, and collaborate with Specialist Nurses, GPs and other practitioners; monitor care standards.
