Monday to Friday – 09:00 to 17:00

Salary

This is a full‑time position; salary up to £55,086 per annum, depending on experience.

Main duties of the job

As a Long Term Conditions Nurse you will:

  • Identify immediate health needs, including any external services or prescribing requirements that prisoners may have received before remand or repatriation onto a wing.
  • Facilitate arrangements between agencies involved in delivering a complex care package to ensure effective collaboration and stability for the patient in delivery of that care.
  • Deliver continuity of care across health and social care agencies from courts, to custody and the community while improving communication.
  • Inspire continuous improvement of MDT working across HMP and healthcare teams.

Requirements
  • NMC‑registered Adult Nurse
  • Post‑graduate experience in Long Term Conditions or equivalent qualification
  • Knowledge of health and social care models across the criminal justice system
  • Experience assessing the needs of and working with individuals with complex health and social care needs
  • Experience in a hospital, community or prison environment is desirable but not essential
