• Assess needs in line with Directorate policies and Working Together to Safeguard Children.
• Develop outcome-focused intervention plans with children, young people and carers.
• Maintain accurate case records according to Directorate policy.
• Review interventions to ensure planned outcomes are achieved.
• Identify unmet needs during assessments and report to managers.
