Contract role – Immediate Care Team, Integrated Care System (ICS)

Team: Immediate Care Team, IPC (Integrated Care Partnership)

Essential Requirements

• Car driver
• Registration with HCPC
• Experience working in multidisciplinary team (MDT) and rehabilitation (preferable)
• Some postgraduate experience rather than just qualified, due to the complexity of the team's caseload at times
• Must have some community experience
• Ability to work across all pathways of Intermediate Care – reablement, rapid response and rehab beds
