Job Summary

Lead the Bristol locality team within Sirona Adult Learning Disability Health Service (ALDHS). Work closely with other locality leads, professional leads and project lead as a key member of the ALDHS leadership team.

Key Responsibilities
  • Lead the locality team in Bristol and collaborate with lead teams across Bristol, North Somerset and South Gloucestershire.
  • Apply clinical skills to complex clinical situations and deliver coordinated and safe multidisciplinary working.
  • liaise with health and social care agencies to facilitate safe care.
  • Promote the wellbeing of the team.
  • Lead referrals triage and advice & guidance processes on a duty basis.
  • implement and oversee quality improvement initiatives.
  • Collaborate with the leadership team in decision‑making processes.
Qualifications
  • Clinical qualification in a relevant health profession.
  • Current registration HCPC or NMC registration.
  • UK driving licence and access to a vehicle during working hours.
Experience
  • Experience working with adults with learning disabilities and complex needs.
  • Experience of providing leadership.
  • Experience of working collaboratively to achieve the goals of the service user and the service.
  • Experience of working across organisational and professional boundaries to improve care.
