It is an exciting time to join the Business Support team as we roll out our Living Well Offer. We are looking for a proactive and hardworking Living Well Officer to join the Norwich Older People/Physical Disability Team in Adults Social Services. NCC is working in a hybrid way and therefore there will be some time working from home and some from the office.

Main duties of the job

The role is customer focused, with the expectation to deliver administrative and support services directly to the Operational Teams and service users. You will be the first point of contact, liaising with Stakeholders, Partners, and Customers, therefore you must possess excellent communication skills. This is an exceptionally varied role supporting multiple teams with involvement in daily tasks such as sourcing grants or community activities, minuting meetings, supporting protection of property requirements under the Care Act, paying invoices, answering phones, or organising food parcels. In addition, you will be expected to maintain a list of local resources that you can draw upon when considering requests for support from practitioners.

You will also make some visits to support the operational team so you will need to be able to drive and have access to a car. The Living Well model seeks to embed our vision to support people to be independent, resilient, and well in the community. The role is key to the delivery of our objectives so you will be busy and will need to be able to manage competing priorities. Successful applicants will have a full induction, receive ongoing training and support and there are opportunities for further career development. Job responsibilities

Job Purpose: to provide a wide range of administrative and support services to Operational teams to ensure a consistent Living Well offer is available across the county. The Living Well Officer will work alongside operational teams to develop local knowledge of existing events and community groups and provide relevant information to teams, health colleagues, district councils, providers, service users and family members.

Context: Living Well is Norfolk’s implementation of the 3 Conversations way of working. It is an approach to social care practice that starts with having open and interested conversations with people and families needing support. The first conversation aims to build links and connections in the community so that people can get back to their lives without formal support. The second, which may not be needed, is used in a crisis to put together an emergency plan without the need for a formal Care Act eligibility determination. The third, which again may not be needed, is about putting together a longer‑term plan that involves formal support, asking the question, "what does a good life look like?" This role will help facilitate the Living Well model by providing an initial point of contact and associated support and solutions. The role will also liaise with providers, district councils, agencies and health partners regarding administrative, contractual and financial arrangements, convening meetings and supporting Living Well outcomes.

  • Good understanding of functions of Microsoft Office
  • Must have a full driving licence and access to a car
  • Knowledge of current policy and practice regarding Promoting Independence and the Living Well model.
