Job summary

Caring Hand Home Health Solutions, Inc. is seeking a per diem Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) to provide direct in-home patient care in California.

You will follow RN plans, educate families, and document care. Candidates must have an accredited LPN/LVN program diploma, active CA license, CPR, 3+ years experience, valid driver's license, and ability to travel to patient homes.

Qualifications

• Graduate of an accredited practical or vocational nursing program.
• Active LPN or LVN license in California.
• 3+ years of nursing experience; home health or med/surg background preferred.
• Current CPR certification with AHA.
• Valid driver's license, reliable insured vehicle, and ability to travel to patient homes.
• Strong communication skills and a collaborative, patient‑first mindset.

Responsibilities

• Provide direct patient care in accordance with the state Nurse Practice Act.
• Implement and follow individualized care plans established by the RN.
• Educate patients and their families on self‑care techniques and diagnosis‑related information.
• Document care accurately and in a timely manner.
• Collaborate with the care team – including OT, PT, HHA, and other disciplines – and communicate patient needs to the RN or Clinical Supervisor.
• Use supplies and equipment safely, adhering to aseptic technique.
• Perform other duties as required for patient services.
