What you will do: You will support people living with long‑term conditions to live well in the community. Working alongside them, their carers and families, you will understand what matters most and provide the support needed to help them live a safe and independent life. Flexibility is important – we offer full‑time, part‑time and flexible working options within an agile hybrid model that includes home working and flexible office spaces.

Benefits
  • Annual starting salary £37,856 (estimated) with a Level 2‑3 pay range of £42,839 – £53,460 (pro‑rata for part‑time).
  • Recruitment and Retention Supplements up to £5,719 per year (pro‑rata for part‑time).
