Job summary

Job Description

Personal Advisor (Care Leavers) Are you passionate about supporting young people as they take their first steps into adulthood? We’re currently recruiting a Personal Advisor (PA) to work with care leavers, helping them build independence and achieve their future goals.

As a Personal Advisor, you will play a vital role in guiding young people aged 16–21 (and up to 25 where applicable) through their transition into adult life, ensuring they have the support, skills, and opportunities they need to thrive.

Pathway Planning
  • Develop, implement, and regularly review Pathway Plans with young people, social workers, and partner agencies.
  • Ensure goals around health, education, employment, and housing are realistic, achievable, and actively supported.
Agency Coordination
  • Build strong working relationships with external agencies, families, and carers.
  • Ensure a coordinated and consistent support network around each young person.
Housing & Financial Support
  • Support young people to secure and sustain suitable accommodation.
  • Work with housing teams and benefits agencies to assess needs and entitlements.
Skills Development
  • Help young people build practical life skills needed for independent living, including budgeting, household tasks, and accessing community resources.
Advocacy
  • Act as a trusted advocate, empowering young people to express their needs, challenge decisions, and make informed choices about their future.
Monitoring & Reporting
  • Maintain accurate records of progress and provide timely updates to ensure effective delivery and review of support plans.
Transition Support
  • Take over support from the social worker (typically just before age 18) to ensure a smooth, consistent transition into adulthood.
