Job summary
Sussex Partnership NHS Foundation Trust in Horsham is recruiting a Lead Practitioner to support a small caseload of clients aged 14-65 with First Episode Psychosis or At-Risk Mental State in the community. You will co-ordinate care from referral to discharge, developing person-centred treatment plans and involving families where appropriate.

Qualifications
- Registered Practitioner with current registration appropriate to the job role
- Evidence of continuous professional development
- Experience of managing a caseload
- Experience of providing clinical interventions for psychosis and related care
- Experience of partnership working within and across teams
- Understanding of CPA process and clinical governance
Responsibilities
- Triage and assess referrals to services
- Plan and implement care and carry out specialist assessments
- Provide specialist advice to the MDT and carers
- Conduct risk assessments and participate in case formulation meetings
- Engage with families and partner agencies to support recovery
- Contribute to supervision and service governance
