Main Duties of the Job
- Leading clinical pharmacy services – setting the standard for safe, evidence-based prescribing.
- Conducting Structured Medication Reviews (SMRs) – optimising medicines for patients with long-term conditions.
- Prescribing and deprescribing confidently – ensuring treatments align with best practice and patient needs.
- Providing expert advice – supporting GPs, nurses, and the wider team with complex medication queries.
- Managing post-discharge reviews – ensuring seamless transitions and safe medication changes.
- Mentoring and supervising pharmacy staff – building capability and confidence across the team.
- Auditing and improving prescribing quality – driving medicines optimisation and governance.
- Leading quality improvement projects – promoting adherence to NICE guidance and local formulary.
- Collaborating across practices and PCNs – working with clinicians to achieve prescribing targets.

Purpose of the Role
To provide senior clinical oversight and leadership for pharmacy services across Penrose Health. This role ensures safe, evidence-based prescribing, optimises medicines management, and supports the development of the pharmacy team through mentorship, supervision, and quality improvement initiatives. The Lead Clinical Pharmacist will act as a clinical expert, driving best practice and improving patient outcomes.
Key Responsibilities
- Clinical Care
- Conduct Structured Medication Reviews (SMRs) and manage long‑term conditions within scope of practice.
- Prescribe, deprescribe, and optimise medicines in line with NICE and local guidelines.
- Provide expert advice on prescribing and therapeutics to clinicians.
- Review patients post‑hospital discharge and manage complex medication changes.
- Support safe prescribing for care homes, vulnerable patients, and polypharmacy cases.
- Clinical Leadership & Supervision
- Provide day‑to‑day clinical supervision for pharmacists and technicians.
- Lead case discussions, peer reviews, and learning from significant events.
- Support induction and ongoing development of pharmacy staff.
- Deliver clinical education sessions and identify training needs.
- Quality Improvement & Governance
- Lead medicines audits and quality improvement projects across sites.
- Ensure compliance with formulary, NICE guidance, and safety alerts.Support achievement of QOF, IIF, and PCN prescribing targets.
- Contribute to development and review of prescribing protocols and pathways.
Qualifications
- GPhC‑registered pharmacist.
- Minimum 2 years clinical pharmacy experience.
- Any relevant leadership or supervisory role.
- CPPE training completed.
