A healthcare provider in Chesterfield is seeking a Band 6 Lead Nurse for the Specialist Memory Assessment & Dementia Support Service. The successful candidate will conduct detailed memory assessments and provide compassionate care for individuals with cognitive difficulties. Specialist Memory Assessment & Dementia Support Service (SMADSS)Following the recent merger of Memory Assessment Services and OA Day Services, we are now operating as SMADSS. We are seeking an enthusiastic Band 6 Lead Nurse to join our dynamic and welcoming team based in North Derbyshire.
Our service runs Monday to Friday, 9:00am-5:00pm (excluding bank holidays). This is a full‑time, permanent position of 37.5 hours per week.
Carry out comprehensive and specialised initial memory assessments for individuals experiencing memory or cognitive difficulties that may be associated with dementia.
Apply person‑centred and compassionate approaches, actively listening and responding to individuals to encourage their engagement with the Memory Assessment Service (MAS).
Use a robust framework for risk assessment, safeguarding, signposting, and referrals to specialist teams, ensuring collaboration with the individual throughout the process.
Provide support across Derbyshire, delivering services in a variety of settings to meet the needs of the Memory Assessment Service.
