A local healthcare provider in Bradford is seeking a Registered Nurse to oversee assessments and deliver personalized commissioning as part of comprehensive care management. The role requires strong clinical leadership and involves collaboration with various health and social care professionals. Undertaking assessments in accordance with the NHS National Frameworks for NHS Children and Young Peoples Continuing Care/NHS Continuing Healthcare and NHS-funded Nursing Care.
Support the investigation of complaints and review of Continuing Healthcare retrospective cases, appeals and independent panel review process.
Support the delivery of personalised commissioning to include transforming care work (Care Programme Approach, Care and Treatment Reviews, Section 117 aftercare arrangements), Personal Health Budgets, the coordination of complex cases (incorporating Deprivation of Liberty Safeguards (DOLS) and clinical input into Court of Protection where appropriate.
Working independently within codes of practice, standard operating procedures and professional guidelines, providing strong clinical leadership ensuring the highest standard of clinical assessment.
Ensuring a patient focused, collaborative approach to identifying health needs and facilitating the most appropriate and cost-effective care pathway for individuals.
The post holder will support complex case management and work in collaboration with other health professionals and social care staff. The post holder will deputise for the band 7 Clinical Leads.
Undertaking assessments and reviews for patients across all aspects of care. This necessitates a high level of assessment skills requiring tact, diplomacy and the ability to liaise effectively with colleagues, health care professionals, service users' carers and family members.
Complete MCA assessments and Best Interest decisions where appropriate. Work within the DoLS framework, identifying and responding to the need for DoLS authorisation. This may include working with legal professionals and the Court of Protection.
Offering continuing support and advice to individuals, families and carers in complex and sensitive situations, necessitating a high level of interpersonal and reassurance skills; supporting the local resolution process in the case of complaints/appeals/retrospective cases and providing information on a timely basis.
Developing and maintaining effective and appropriate communication networks with health and social care professionals and act as a resource to them.
Working with relevant health professionals, colleagues, and key stakeholders to ensure a seamless and efficient service is delivered.
Exercising professional judgement as necessary when analysing and communicating complex and sensitive information on behalf of the Team.
Interpreting national and regional guidance relating to the implementation of the NHS National Frameworks and subsequent developments within service delivery.
Decision making following multi-disciplinary team assessments to determine eligibility.
Deputise for the Clinical Lead when required.
Deliver against organisational objectives, achieving quality outcomes, prioritising own workload and working to tight deadlines.
Plan, organise and implement reviews and assessments, to co‑ordinate with local authority colleagues, key stakeholders, individuals, families and carers.
Ensuring there is a holistic care plan in place, including the production and co‑production when required with health and care partners.
Ensure personal health budgets are actively promoted with individuals and their representatives and work closely to encourage a seamless delivery.
Identify risk of abuse and need for protection from harm to self or others and report in line with local safeguarding routes and governance arrangements.
Ensuring timely end of life provision and reviews, following the national guidance for clients fast tracked through the CHC system.
Providing specialist nursing advice to secondary care, primary care, local authority partners and other key stakeholders to facilitate a seamless process.
Ensure policies are implemented and adhered to whilst identifying and proposing changes for improvement to practices and procedures in own area.
Approving a package of care in line with Place‑based Partnership standing financial instructions and agreed thresholds.
Continually strive for delivering project/function outcomes, value for money and greater efficiency.
Agreeing eligibility and will make financial decisions within the Place‑based Partnership standing financial instructions.
Delivering formal and non‑formal education programmes to community and hospital staff, including nursing/residential homes, GPs, community nursing and therapy staff relating to the assessment and management of Continuing Care/Continuing Healthcare patient groups in line with the latest research, evidence and Department of Health/NICE guidance.
Maintaining and updating clinical records in a timely manner and in line with local and national policies.
Treat people with dignity and respect.
Maintain a high standard of professional and personal conduct.
