Oxleas NHS Foundation Trust in Greenwich is seeking a Band 6 clinical post in the Home Treatment Team. This role involves complex assessments to decide if crisis care can be provided at home, with seven days a week, year-round service and 72-hour post-discharge follow-up.

The post requires professional registration and HTT or community mental health experience, with responsibilities including leadership of junior staff, risk management and collaboration with families and partner agencies.

Responsabilités

• Gatekeep acute inpatient admissions by completing complex assessments to decide if patients can be treated at home.
• Deliver intensive short-term clinical work in the Home Treatment Team and 72-hour follow-up after discharge.
• Provide clinical leadership, mentoring junior staff to deliver safe, recovery-focused care.
• Lead and coordinate teams in challenging situations using risk management and de-escalation.
• Act as Shift Coordinator when required, managing workload and referrals.
• Carry out mental health assessments, MSE, risk assessments and care plans.
• Work with service users, carers, multidisciplinary teams and partners on person-centred care.
• Manage medication, monitor physical health and provide health promotion.
