Sheffield Teaching Hospitals NHS Foundation Trust is seeking an experienced Diabetes Specialist Nurse (Inpatient Team) to develop advanced specialist skills in diabetes care. The post provides leadership within a multidisciplinary diabetes team and supports high quality patient pathways in secondary care.

You will manage a defined caseload, deliver diabetes education, and work with autonomy to improve services, contributing to audits and ensuring smooth transitions along the patient journey.

Qualifications

• Degree level education or working towards.
• To have completed the national practice assessor training or equivalent teaching and assessing qualification for those with ENB998 or equivalent experience in delivering diabetes education
• Independent nurse prescriber or equivalent or willing to work towards
• Evidence of advanced training in diabetes

Responsabilités

• Manage own defined caseload of patients with highly complex or chronic conditions, providing specialist advice across professional boundaries and planning and implementation of care.
• Run nurse led clinics and comprehensive follow up services; ensure continuity of care.
• Provide diabetes centre cover, offering urgent appointments and advice to patients and primary care providers.
• Develop service improvement through audits and evaluations to benefit patient care.
