South Warwickshire University NHS Foundation Trust is seeking a Specialist Palliative Care Clinical Nurse Specialist to join the community team. The role is based at Whitnash Lodge, Warwick, with weekend rota duties across the county and cross-County working as required.

The post holder will work within the Specialist Palliative Care Team across Warwickshire, supported by a Consultant, and will lead nursing care in partnership with the MDT to deliver high-quality patient outcomes.

Competenze

Registered Nurse, NMC-registered with relevant post-registration experience in palliative care.
First level degree in related subject; Masters level or willingness to pursue.
Post-basic qualification in palliative care and mentors/assessors qualification.

Deliver evidence-based personalised and compassionate care within Specialist Palliative Care.
Manage a clinical caseload with high autonomy and advocacy.
Develop and evaluate care pathways with the MDT to improve patient outcomes.
Provide expert nursing leadership and contribute to education and service development.
Support weekend on-call duties and contribute to clinical governance.
Lead and participate in teaching and mentoring of junior staff.
