Job summary

The post holder at Betsi Cadwaladr University Health Board in North Wales will provide leadership as a Specialist Community Co-Occurring Practitioner, working across Conwy CMHT and Substance Misuse Services.

You will lead the Co-Occurring Framework, maintain a clinical caseload, and drive service improvement through audits, training and robust governance, in collaboration with multidisciplinary and third-sector partners to deliver person-centred care.

Qualifications

• Experience in specialist community working with co-occurring mental health and substance misuse needs.
• Leadership in multidisciplinary settings and governance.
• Ability to develop and monitor integrated pathways and care plans.

Responsibilities

• Lead the implementation and monitoring of the Co-Occurring Framework within CMHT and SMS.
• Maintain a clinical caseload while delivering high-quality care.
• Deliver Training to improve clinical practice and drive service improvement.
