Job summary

Southern Health NHS Foundation Trust in Basingstoke is seeking a Band 6 Community Mental Health Practitioner to join the Older Persons CMHT. The role involves assessment, diagnosis, treatment and support for older adults with cognitive decline, dementia, or functional mental health issues.

You will work with carers and other services. Based at Parklands House, London Road, you will manage a caseload, contribute to duty rotas, supervise staff, and produce reports.

Qualifications

• Sound clinical skills and ability to coordinate complex care.
• Excellent assessment and communication abilities.
• Good administration, IT skills and report writing.
• Knowledge of safeguarding procedures.

Responsibilities

• Carry a caseload within the Older Persons CMHT.
• Coordinate care with other agencies and services.
• Produce reports and supervise staff as required.
• Participate in duty system and run clinics as needed.
