Job description

Pharmacists are an essentialpart of the current and future NHS workforce and as such an exciting opportunity has arisen for an experiencedClinical Pharmacist to join our supportive and forward thinking Practice.

As LeadPharmacist you will offer clinical leadership to the existing Pharmacists andmentorship to the Pharmacy team and Pharmacy Technicians. You will beinstrumental in tailoring the design and delivery of pharmacy interventions andpolicies that address Practice and Bassetlaw wider health and care prioritiesand ensuring that the medicines management processes within the practice areoptimised to achieve effective, safe, and patient centredprescribing. You will hold an independent prescribing qualificationand will undertake patient consultations as appropriate with areas ofresponsibility that include chronic disease management, patients with complexpolypharmacy and patients with multiple long-term conditions.

Main duties of the job

The main duties of the roleare:

  • To lead and develop the Pharmacy team
  • To ensure safe, effective, and efficient medicines management processes are in place
  • To undertake a patient facing role andundertaking clinical medication reviews to proactively manage patients withcomplex polypharmacy
  • To provide pharmacy advice and guidance

Experience
