Job summary

You will produce and review care plans, monitor delivery to required standards, liaise with health services, families and advocates, and provide direct care in emergencies when needed.

Qualifications

• Knowledge of rota planning to ensure adequate staff with correct skill mix.
• Experience conducting Client Introductory visits.
• Experience working with service users who have additional support needs.

Responsibilities

• Produce and review care plans for all service users in accordance with procedures.
• Monitor the delivery of care to meet required standards.
• Update care plans and maintain accurate records.
• Provide direct care in emergencies to ensure appropriate care packages.
• Assist service users to access specialist care where needed.
• Encourage daytime activities and social engagement.
• Liaise with health services, professionals, families and advocates.
• Administer and supervise medication according to policies.
• Attend supervision, appraisals and meetings.
• Support on-call rota and act as OOH contact.
