Job summary

NHS Surrey and Sussex ICB is seeking a Learning Disability and Autism Programme/MH S117 Lead Practitioner (NHS AfC Band 7) to lead reviews and coordinate care for adults with LD and/or autism in inpatient and community settings.

The postholder will chair Care and Treatment Reviews, develop full care pathways, collaborate with local authorities and providers, and drive improvements in access, transition planning and service specification development.

Qualifications

• Educated to degree level or equivalent in a relevant subject
• Experience of working with people with learning disability, autism and mental health needs
• Knowledge of MHA s117, Mental Capacity Act and Care and Treatment Reviews
• Experience of managing a team or mentoring staff

Responsibilities

• Chair and lead Care and Treatment Reviews (CTR) for adults with LD/autism in inpatient and community settings
• Coordinate and facilitate Local Area Emergency Protocol Meetings
• Support discharges from Mental Health Inpatient Units
• Collaborate with Local Authority and providers to develop full care pathways
• Collect and analyse data for key projects and boards
• Provide training and guidance to internal and external partners
