Job summary

Essex County Council is seeking a Case Manager to support the Transforming Care Programme for Autistic individuals and those with learning disabilities. This role is pivotal in driving health and social care improvements ensuring timely discharge from mental health beds and community support.

Applicants should have a relevant degree, substantial experience in the field, and strong communication skills. The position requires travel across Essex, and we offer flexible working to support the needs of our employees.

Qualifications

• Significant experience with learning disabilities and/or autism.
• Up-to-date understanding of relevant legislation including the Mental Health Act.
• Ability to chair meetings and make recommendations in complex situations.

Responsibilities

• Provide specialist advice and expertise for health professionals.
• Chair Care and Treatment Reviews for clients.
• Facilitate timely discharge from inpatient services.
